It is the goal of the Piedmont Triad Volleyball Club to turn no one away because of inability to pay. Contributions to the Financial Assistance Program help to provide financial assistance on a sliding scale and to keep our program fees affordable.

Click on the link to the left and when you have the choice of Open or Save, select Save and save it to your computer.  It is a Microsoft Word Form file where you just fill in the fields and then you print it out and mail it to the club.
