Registration Checklist

  1. USA Volleyball Membership Form and Fee
    bulletRegister Online.  Follow directions.
    bulletMake sure birth date is correct (affects age group).
    bulletMake sure to include High School Graduation Year.
    bulletMake sure to print off signature form and sign (player and parent signatures)
    bulletWrite your check or pay online for the membership fee to: Carolina Region USAV
    bulletMail your signature form and check to:

    Carolina Region USAV, P.O. Box 1757, Clemmons, NC 27012

  2. Fill out online Piedmont Triad Volleyball Club Information Form.
  3. Medical History and Release Form
    bulletMake sure to write legibly.
    bulletMake sure information is accurate.
    bulletMake sure the form is signed on the front by both the player and the parent/guardian (pay attention to where you sign - one is authorizing medical care and one is not).
    bulletYou do not have to have the form notarized.
    bulletMake sure the information on the back is filled out accurately and completely.  Put none or NA instead of leaving blank.
  4. Photocopy of Birth Certificate.
  5. Pay Tryout Fee.
  6. Pay Deposit (separate check) to be considered for a team.

Other payments due

bulletPay Greensboro Sportsplex fee by the first practice.
bulletPay club balance due or first payment of the payment plan by December 15th, 2007.
